By affirmation of this disclaimer and by acceptance of its Terms of Use, I give concent and release any First Responder, Medical Personel, and/or any individual or individuals regaudless of their level of medical training from any liability when accessing the information stored within MyMedicTag when used to render aide and assistance to the Owner of the Tag or any of the Owner's subscribers in the event of an accident or medical emergency.
2. I acknowledge that First Responders ans/or Medical Personel are under NO obligation to use MyMedicTag. The use of MyMedicTag is at the sole descretion of First Responders or Medical Personel based on the severity of the situation. St.James LifeScience Inc. waives all responsibility and defers to the First Responders and Medical Personel's descision as final.

I acknowledge the sensitive nature of the information that I will enter into MyMedicTag and as such I realize that it is My responsibility yo safeguard my informationat ALL times to the best of my ability. I also waive any claims against St.James LifeScience Inc., its officers, employees, or subcontractors.
4. I understand that it is at my descretion that I freely enter as much information and detail as I feel comfortable. I further un derstand that the omission of some information may delay the timely dispensing of my medical care and treatment.
